What information do we collect?
You have the option to provide Protected Health Information or Personal Health Information (PHI) to us. If you choose to provide your information, we will collect your (or the patient’s) name, age, health status, and other details about the insurance complaint. The Kennedy Forum is collecting complaint information in order to advocate for change and examine trends in insurance claim denials; any PHI collected for this purpose will be anonymized.

You are not required to provide this information and you may opt out at any time by following the directions provided on the Website.

We may also collect, retain and use personal information you submit ( such as your name, address, telephone number, email address) when you register for a webinar, request a research paper, fill out an online survey, submit a request for information, or contact us.

We do not store any of your personal data in cookies.
Data and How We Use It
This Website is designed to help consumers, providers, advocates, and others learn how to file a complaint with a health plan or regulatory agency when care is denied for mental health and addiction services. We also will conduct research to study trends both at the state and national level with the goal of improving mental health and addiction insurance coverage.

The information you provide will not be sold or used for commercial purposes. If you submit PHI (such as your date of birth or a medical condition that can be associated with you), we will used and disclose that information only for the purposes stated above and strictly in accordance with applicable federal and state privacy laws.

If you are a HIPAA covered entity or a federally-assisted substance use disorder treatment program covered by 42 CFR Part 2, please do not submit any PHI regarding your patient until you have entered into a Business Agreement and Qualified Service Organization Agreement with the Kennedy Forum.

We will not sell or distribute your information to a third party. You may opt out of future communications from us by contacting us or by unsubscribing from any communication that we send you.
How Do We Protect Your Information?
We encrypt your sensitive information using secure socket layer technology (SSL). We follow industry accepted industry standards to protect the personal information submitted to us, both during transmission and upon reception. No method of transmission over the Internet or method of electronic storage is 100% secure, however. Therefore, while we use strong commercially acceptable means to protect your personal information, we cannot guarantee its absolute security.
